Home Visiting Program

Siskiyou County Public Health offers two home visiting programs to those eligible for services.  Families are paired with a home visitor who is a nurse or trained professional who learns more about the family’s needs and strengths and makes regular home visits to provide parenting support.

Both programs are Healthy Families America (HFA) Home Visiting Programs that are provided at no cost, and enrollment is always voluntary.

Healthy Families America is a national home visitation program that provides expectant and new parents, or caregivers the education and support they need from pregnancy and through the first years of their child’s life. HFA is one of the leading family support and evidence-based home visiting programs in the United States.

The Home Visiting Programs nurture healthy parent-child relationships and include benefits such as:

• Information and tools for nutrition, prenatal care, infant and toddler care
• Support for mental health, domestic violence, or substance abuse
• Child development screenings & information
• Family Goal Planning
• Access to community partnerships and resources.

CalWORKs Home Visiting Program

The purpose of the CalWORKs HVP is to support positive health, development, and well-being outcomes for pregnany and parenting people, families, and infants born into poverty through evidence-based home visiting models.

To be eligible for home visiting services per W&IC section 11330.6(c)(2), a voluntary participant shall meet both of the following criteria:

The individual is:

(I) The individual is either pregnant, or the parent or caretaker relative of a child less than 24 months of age at the time the individual enrolls in the program; and

(II) (i) A member of the CalWORKs assistance unit; or

(ii) The parent or caretaker relative of a child-only case; or

(iii) An individual who is apparently eligible for CalWORKs aid.

California Home Visiting Program

The California Home Visiting Program (CHVP) is designed for overburdened families who are at risk for Adverse Childhood Experiences (ACEs), including child maltreatment, domestic violence, substance use disorder, and mental health-related issues.

• This program is available to parents who meet income eligibility requirements.
• Services are offered to pregnant mothers and parents or caretakers of a child under 3 months old.
• Services are provided to the families for up to 3 years.
