Diabetes Empowerment Education Program (DEEP)
The Diabetes Empowerment Education Program, also known as DEEP™, is an educational course designed to help people gain a better understanding of diabetes prevention and self-management. Classes last a total of six weeks, providing participants with eight unique learning modules.
Eligibility
- At risk for diabetes
- Have been diagnosed with pre-diabetes
- Are diagnosed with Type 2 Diabetes
- Are helping a family member or friend manage their diabetes
Program Goals
- Improving and maintaining the quality of life of persons with pre-diabetes and existing diabetes
- Preventing complications and incapacities
- Improving eating habits and maintaining adequate nutrition
- Increasing physical activity
- Developing self-care skills
- Improving the relationship between patients and healthcare providers
- Utilizing available resources
Program Details
- In-person and Interactive
- 6 Week Commitment
- Class time is 1 ½ to 2 hours
- Free to all community members
