Diabetes Empowerment Education Program (DEEP)

The Diabetes Empowerment Education Program, also known as DEEP™, is an educational course designed to help people gain a better understanding of diabetes prevention and self-management. Classes last a total of six weeks, providing participants with eight unique learning modules.

Eligibility

  • At risk for diabetes
  • Have been diagnosed with pre-diabetes
  • Are diagnosed with Type 2 Diabetes
  • Are helping a family member or friend manage their diabetes

Program Goals

  • Improving and maintaining the quality of life of persons with pre-diabetes and existing diabetes
  • Preventing complications and incapacities
  • Improving eating habits and maintaining adequate nutrition
  • Increasing physical activity
  • Developing self-care skills
  • Improving the relationship between patients and healthcare providers
  • Utilizing available resources

Program Details

  • In-person and Interactive
  • 6 Week Commitment
  • Class time is 1 ½ to 2 hours
  • Free to all community members
