Five Wishes
SCPH is pleased to offer no-cost educational presentations about advance care planning using the Five Wishes curriculum.
Five Wishes is a resource to help start and guide your advance care planning conversations and help you document your choices as a legal advance directive.
Advance Care Planning is making decisions in advance about medical care and treatments at the end of life, and choosing a decision-maker who will speak for you if you ever become unable to speak for yourself.
The best time to have these conversations is before a crisis. Documenting your wishes in an advance directive is an important part of the process.
