Below are printable Partnership Provider Directories. Printed directories are updated every 30 calendar days. For the most current information, please use the searchable directory.
You may print selections directly from the searchable directory or you may request a copy of a Provider Directory by contacting our Member Services Team at (800) 863-4155. TTY Users: Call the California Relay Services at

Find an error in the Provider Directory?
The online directory is updated daily to reflect changes made the previous business day. If you believe that you have found an error in the Provider Directory, send an email to PHCDirectory@partnershiphp.org including your first name, last name, and phone number in case there are questions on the error or call us at (800) 863-4155.
Providers, you now have the ability to verify your site's information in the online searchable directory. Once you select "show details" after searching for your site, there will be a verification box on the bottom right hand corner. You can also submit requests to update information at the same time.
